New Patients

Our patient registration area covers Bethesda and the surrounding area of approximately 6 miles.

How to register

Please come to the reception desk and ask for a registration form (GMS1W) which you can complete at the desk or take home with you and return later. Please complete the form as fully as possible (the receptionist will assist you if required). Once this form is complete the registration process starts and we will be able to obtain your medical records from your previous GP.

The GMS1W Registration Form MUST been signed in the bottom left corner of the front page. We cannot accept it unsigned.

Just Visiting

If you are visiting and not usually resident within the Practice catchment area you may register as a temporary resident should you require to see a doctor. Please come to the reception desk and ask for a temporary resident registration form. The receptionists will assist you with the form and in making an appointment.

Change of Address or Telephone Number

Please let us know promptly of any change of address or telephone number.  It may be important in an emergency and will ensure, should we write to you or call you regarding an appointment, that you are contacted quickly.
